Each person will be screened at the door. Only individuals who answer “no” to the following questions are permitted to enter:
Have a fever of 100.4 or higher
Have a cough and other respiratory symptoms such as shortness of breath
Been in direct and prolonged contact with COVID-positive individuals in the last 14 days
Only patients are permitted in the building (unless patient is a minor in which case one guardian may accompany). We ask that family, friends, or rides wait in the car.
We kindly ask that you not arrive more than 5 minutes ahead of your scheduled appointment time, unless directed by one of our team members to do so.
We are recommending that all patients wear a mask while in the building. If you are unable to wear a mask for any reason, reasonable accommodations can be made. Please alert one of our team members to discuss options in this situation.
All New Life Physical Therapy team members will be wearing a mask when working directly with patients.
We ask all individuals to wash their hands or utilize hand sanitizing stations upon entry and exit of our clinic.
We have increased cleanings where we wipe down all commonly touched items in passing. We wipe down and clean each piece of therapy equipment and items after use or contact.
Waiting areas have been redesigned to allow for social distancing. You may choose to wait in your vehicle until your therapist comes to get you. Please call us at 608.742.9356 (Portage) or 608.356.2334 (Baraboo) to let us know you have arrived.
Magazines, newspapers and coffee stations have been temporarily removed. Guest WiFi access is available for you to utilize while you wait, as well as tv monitors for information, updates, and entertainment.
Opportunities to complete paperwork online and ability to make payments over the phone or place credit card on file are encouraged to limit any further contact.
We are no longer accepting cash. We accept credit cards, health savings and flex spending cards, and checks for visits, co-payments, deductibles, supplies, etc.
