How does it work? Based on how you answer questions on your initial evaluation, you are given a current status score, a projected improvement score, with projected amount of visits and time it will take to achieve this goal. Your score takes into account specific information about YOU! Including age, medical history, and your current condition for which you are being treated. Your therapist takes this tool into account when developing and discussing your plan of care for physical therapy.
Completing the Survey: Every few visits or as you experience change in your condition/status, you will be kindly asked to complete and update your status. This will take just a few minutes and will be done on our clinic’s tablet. If you require any assistance, please do not hesitate to let us know. We are here to help in any way we can.
Your Score: Your score and results will be shared with you by your therapist after every report you complete. We invite you to be active in tracking your own scores and ask any questions along the way.
What is this information used for? This information is used to help your therapist plan treatment sessions, set and revise plan of cares, and determine appropriate discharge time. Additionally, it is helpful information sent to insurance companies to help ensure that your physical therapy services are covered under a medical necessity plan of care.
