Bryan is a physical therapist that practices at the Baraboo and Portage locations, and onsite at New Life For Work industrial locations. Bryan earned his Master of Science degree in Physical Therapy from Des Moines University in 1999. Prior to his physical therapy education, Bryan attended South Dakota State University to study exercise physiology, earning a Master of Science in Health Physical Education and Recreation. Bryan’s post professional education has included manual therapy (certified through the North American Institute of Orthopedic Manual Therapy), Applied Functional Science (fellowship trained through the Gray Institute), and treatment of myofascial pain and trigger points (certified through Myopain Seminars). Bryan is Vice President of New Life and Clinic Director of the Baraboo location.
What Bryan likes most about being a physical therapist:“I have the opportunity and ability to help my patients and clients do things they didn’t think were possible. What is now possible may include the fun activities they feared were lost forever to an old injury or a chronically sore body part.”
Outside of work: Bryan really enjoys weight lifting, weight training, fishing, golfing, boating, and skiing.
Jamie Bockhop PT, FAFS, ATC Physical Therapist

Jamie is a full-time physical therapist practicing in the Baraboo clinic and onsite at various industrial locations. He earned his Master of Physical Therapy degree from UW-Madison in 2006. His post-graduate certifications include Dry-Needling, AFS (Applied Functional Science), and NG360 Nike Golf Performance Specialist.Â Prior to his physical therapy education, Jamie attended UW-Madison earning his degree in Exercise Science and fulfilling his Athletic Trainer requirements while working and traveling with the UW Athletic programs.
Jamie’s current work specialty and professional interests are outpatient orthopedic and industrial rehabilitation. In addition, Jamie has experiences in Athletic Training at the high school, collegiate, and professional levels. These experiences have encompassed a broad range of health conditions, athletic skill levels, personal goals/motivations, age groups, and functional expectations. Jamie has found each client or patient to be unique and wants to use his broad skill set and experiences to aid them in maximizing their potential and personal goals.
What Jamie likes most about being a physical therapist: “Each person is unique and therefore each person’s goals and expectations are different. I enjoy understanding what is important to each person and the smile and satisfaction that comes to them upon achievement. I especially enjoy when a patient or client has the realization that exceeding their initial expectations is often possible.”
Outside of work: Jamie enjoys outdoor activities (yardwork, hiking, going to state parks, and seeing new places with outdoor themes).
Donna Smith PTA Physical Therapist Assistant

Donna is a physical therapist assistant practicing at our Baraboo location. Donna has a bachelor’s degree in Physical Education with an emphasis in exercise science, and she has taken additional courses in basic and advanced athletic training. She worked in private practice PT clinics for five years as a PT Aide with an emphasis in exercise instruction and received her associate’s degree from Northeast Wisconsin Technical College in 1998 as a Physical Therapist Assistant. She has worked in an outpatient setting for 10 years and in a skilled nursing setting for eight.
What Donna loves most about being a physical therapist assistant: “Meeting new people and continuously learning ways to help them.”
Outside of work: Donna enjoys spending time with family and friends, especially lake time in the summer, and bike riding – preparing for her annual bike ride across Iowa.
Alison Prawdzik Patient Services New Life Team Member Since 2011
Ali is a patient services specialist working at the Baraboo location. Her area of focus includes scheduling, welcoming and preparing for new patient arrival, initial insurance setup, front office reception duties, working with patients on updated FOTO surveys, and customer service.
What Ali likes most about working at New Life Physical Therapy/with patients: “One of my favorite things about working in a physical therapy office is watching the progress that a patient makes at each visit. Patients often arrive at their first visit using some sort of walking aide (cane, crutches, walker) or needing to use the lift (elevator) to get upstairs and to see them progress to walking up the stairs on their own is such a huge milestone for the patients. I love being there to experience that with them and encourage them to keep moving forward.”
Outside of work:Ali enjoys spending time with her family and close friends. She also enjoys caring for her pets and walking her dog, Summer. She loves spending time outdoors, being around and riding horses, and Morel mushroom hunting. She also really enjoys going to antique flea markets, stores and thrift stores in search of treasures.
